IMPORTANT CHANGES to how we handle your personal data
NHS Care Data
We will soon be required by NHS England to supply your personal and confidential medical information to the Health and Social Care Information Centre (HSCIC).
This information will be taken from the practice in a form which can identify you. The practice has no choice but to allow the HSCIC to extract this information. However, individual patients can instruct their practice to stop the transfer of their data.
NHS England Care Data
The NHS has commissioned the Health & Social Care Information Centre (HSCIC) to extract personal confidential patient data to enable them to plan and improve services and compare different areas.
This health information along with details such as your date of birth, postcode and NHS number but not your name, are sent to a secure system where it can be linked with other health information.
You have a choice. If you are happy for your information to be used in this way you do not have to do anything.
If you do not wish information about you to be used in this way, please download, print and complete the Care Data Opt-out Form and return the completed form to reception. Alternatively, pick up a form at reception.
Please note, your access to healthcare and the care that you recieve will not be affected by either decision.
